ALUMNI TRANSCRIPT REQUEST – Except as provided by law, records are only released upon prior written consent of the student or the student’s legal guardian.* Per Board policy transcripts cannot be released until all fines and fees have been paid. Please allow 3-5 school days from the date of receipt to process requests.
There is a non-refundable $3.00 fee for each record request form completed.
It is your responsibility to know the type of transcript and delivery method accepted to where you are having it sent as well as to provide the correct and complete delivery information.
CURRENT STUDENTS-If you are a current Cloverleaf High School student DO NOT use this ALUMNI TRANSCRIPT REQUEST site. Current CHS Students can request transcripts through their NAVIANCE accounts. If you are not sure how to do this, please contact your School Counselor.
*If you are a service center, college, or other entity requesting a transcript on an alumni's behalf, you must email the records release authorizing you to receive the transcript on the alumni's behalf to [email protected] before the transcript will be released to you.

Official Sealed transcripts can be sent ONLY through USPS mail service or picked up. Official Sealed transcripts CANNOT be sent through email.
