Once
the form is completed, please upload it here. We must have a physical
card on file for your student-athlete prior to him/her being allowed to
participate in practice and events.
All student-athletes in grades 6-12 must have a completed physical form on file with the school prior to participating in practice or games. Please have your physician fill the form out completely and then upload it for the school's records.
