Dear Parent / Guardian:
This is to notify you that our class will be taking a field trip, and we would like to have your child accompany us:
Date of Trip: Tuesday, June 25, 2019
Place/Location: Larkfield
Drop Off Time: 11:30 am Pick Up Time: 2:30 pm
Transportation: Parent (carpooling encouraged)
Cost Per Student: $47.00
Would you kindly complete the online Field Trip Permission Slip on or before June 11, 2019. Questions should be directed to Ms. Ahrens (Room 330) or Ms. McGrath (Room 204). We also ask that you submit your payment under the Register button on the right side of this website.

3. Confirmation will appear
General Guidelines for students and Families:
Students' financial obligations to the school (lost or damaged lock fees, lost textbook/calculator fees, library books, etc...) must be satisfied before a student may attend the Moving-Up Dance.
- Parent drop off - please enter Larkfield from Clay Pitts and drop off at the main door. Follow the traffic pattern. Car pooling would be beneficial and appreciated.
- No backpacks or bags will be permitted at the dance.
- Students are expected to attend the entire dance from 11:30 a.m. - 2:30 p.m. No students will be released earlier than 2:30 p.m.
- Students will be chaperoned by teachers during the dance. All students are expected to conduct themselves as they would at any school sponsored function. Students unable to abide by the OMS Code of Conduct will be removed from the dance pending parent notification and pick-up.
- Pick-up is scheduled for 2:30 p.m. Please be on time. We ask that parents enter Larkfield and sign out their children.
