7th and 8th Grade CMM Football Payment 2019
Information and full payment must be submitted by July 22, 2019. The Medical Form must be printed, completed by a physician, and turned in to the CMM office by 3:00 PM, July 22, 2019. Players will not be allowed to practice if payment and information have not been submitted.
Your submission of information and payment is your electronic signature for the HOLD HARMLESS AGREEMENT.
The 7th and 8th Grade CMM Football fee of $200.00 covers jersey, pants, socks, t-shirt, helmet decal, and other expenses.
