CMM 6th Grade Football Team
Full payment must be submitted online by August 9th, 2019. The Medical Form must be printed, completed by a physician, and turned in to the CMM office no later than August 9th by 3:00 PM.
Players will not be allowed to practice if payment and information have not been submitted.
