By checking this box, I acknowledge I have read and understand all of the terms and conditions and the responsibility of having a parking permit at Lake Middle High School:

Students will receive a color-coded tag for the designated lot and SPACE. Students must park in the designated lot and parking SPACE and are subject to consequences if they fail to meet all school parking expectations.
2. Permit tags must be displayed in the vehicle at all times.
