Booster Club Fundraiser Commitment Acknowledgement
I acknowledge my Commitment & responsibility for Starting, Completing
and Providing all required documentation for Fundraiser (list of ALL
giveaways, how many items did not sell, invoices and copy of deposit slips
from bank). Fundraiser must be completed within 2 weeks after selling
has stopped.
NOTE: Fundraiser cannot begin until this document has been completed
& signed. After fund raiser has been approved, you will receive a copy
back from Bookkeeper.
