In consideration for my child's participation in the Olentangy High School Cheerleading Winter Program, I hereby agree and promise I will not hold the camp, coaches, the Olentangy Local School District or its employees responsible for any loss, damages, or personal injuries that my child may receive as a result of participation. In the event of any injury, if attempts to contact me are unsuccessful, I give my permission for the camp director to act in my absence for medial treatment.
